Setting up is Simple!
There are three (3)
ways to get your information to us: By Faxing them (available 24
hrs), by mailing them, or if you are in our zoned area we can pick
your information up weekly.
What we will need from
you:
1. Patient Information
Sheet
2. both sides of the
patients insurance card(s)
3. daily superbill.

Explanation of Benefits from insurance carriers
The checks
will still come to you, you will just fax us a copy of your EOB
(Statement received with checks) and we will post them to give you
monthly reports.
